Acupuncture Treatment in Erectile Dysfunction by Shockwave Therapy: A Case Report in an Acupuncture Specialized Health Care Center in Bangladesh

Abstract: Erectile dysfunction (ED), which is often known as impotence, is a kind of sexual dysfunction in which the penis fails to erect or remain erect during sexual activity which is the most common sexual dysfunction among men. Because of its relationship with self-image and problems in sexual encounters, erectile dysfunction may be psychologically devastating. Physical reasons may be established in around 80% of cases. Cardiovascular illness, diabetes, neurological issues, hypogonadism, pharmacological side effects, and hypogonadism are all possibilities. Impotence is caused by thoughts or sentiments in around 10% of instances. In our study, we have reported a complaint from a male patient who was 43 years of age and complained of having difficulty in erection for the last 2 years. He had a medical history of Hypertensive for 4 years and control on drugs. We started treatment by doing a clinical examination of the patient. And it was clinically determined that the patient had erectile dysfunction. We have conducted Acupuncture on the pubic region as well as low-intensity shockwave therapy, per session, weekly. The patient's condition began to improve after the 12th no of session. It was the very first successful instance of Erectile dysfunction treated with Acupuncture and Low intensity shockwave therapy that had ever been documented. The available evidence for the ability of acupuncture to improve ED is insufficient and previous studies have failed to show the specific therapeutic effect of acupuncture for treating ED. But according to our findings in this case report, acupuncture and shockwave therapy might be the most effective treatment for ED. More case reports with different complications and different ages of patients can concrete the demand of acupuncture success in Erectile Dysfunction of men in the future.

Abstract: Ewings sarcoma is a malignant mesenchymal tumor, extremely rare in patients under 10 years of age, being more frequent from the second decade of life. Its annual incidence is low, 1 to 3 cases per year per million children under 15 years of age. The most affected bone sites are: metaphysis of the long bones, Followed frequently by the pelvic bones from 20 to 25%, tumors that affect the axial skeleton have a worse prognosis and affect individuals close to adulthood. The symptoms are common to other bone tumors: intermittent pain and inflammation located in the affected area, and metastases via the hematogenous route to the lung. It appears in patients in the stage of life of greater exercise and changes in the body typical of adolescence. Treatment is based on extensive surgery of the lesion with free section margins whenever the site or location of the tumor allows it, chemotherapy with high-dose drug association, and radiotherapy. More than 90% of these tumors are characterized by a reciprocal translocation of chromosomes 11 and 22, resulting in a novel EWS-FLI1 fusion protein. Because these tumors show only neuroectodermal markers of differentiation, it has been suggested that they arise from neural crest cellsf. Our goal is to present a case of a patient of only two years of age with stage IV disease at the time of diagnosis, highlighting the value of multidisciplinary teamwork where imaging studies and pathology immunohistochemical techniques play a fundamental role in the diagnosis of the entity.

Abstract: Our purpose was to evaluate the uveal metastasis causing progressive vision loss in the advanced rectal cancer. In our case, 34 years old male patient has consulted to our clinic by medical oncology department because of right blurred vision. He was primary mucinous rectum cancer patient and also has lung and vertebral metastasis. The most common intraocular malignant tumors are uveal metastasis. Especially in severe and advanced cancer patients, there may be seen some metastasis and approximately 10% of these metastasis can be seen in choroid. On the other hand, first sign of a metastatic primary tumor can be an uveal metastasis, that’s why it is important to research for primary origin. Extraocular malignant cancers may metastate intraocular, but this event is rare. Intraocular metastasis can be seen in the uveal tract, mostly in the choroid and they are the most common type of intraocular malignancies. Choroidal metastasis is seen like a creamy white or pale yellow mass associated with subretinal fluid. The most common primary origins are breast cancer in female and lung cancer in male patients. They can be diagnose by ultrasonography (USG) and optical coherence tomography (OCT). Intraocular biopsy is useful in patients with clinical suspicion of uveal metastasis but no evidence of primary malignancy despite systemic evaluation. If systemic treatment is not enough to supress the ocular tumor, local therapies such as external beam radiation therapy (EBRT), photodynamic therapy (PDT), and transpupillary thermotherapy (TPT) may be tried. Unfortunately, the survival rate of these patients is low after diagnosis.

Abstract: Introduction: Cyanotic congenital heart diseases includes congenital structural anomalies of the heart with shunting resulted in mixing systemic venous blood into the arterial circulation. Two types of surgical therapies are available for the treatment of this anomalies: corrective and palliative. The goal of palliative surgery is to increase the pulmonary blood flow via systemic-to-pulmonary arterial shunting as the conclusive treatment or as a bridge to corrective surgery. Objective: Obstruction of palliative systemic artery-to-pulmonary artery shunts is one of the causes of deterioration in the clinical status, mortality, and morbidity of patients with cyanotic heart disease. For patients contraindicated for the Fontan surgery, a solution is a new shunt insertion or interventional procedures on stenotic shunts. Redo surgery is associated with higher risks of mortality and complications. Method: Herein, we describe 23 patients with obstructed or stenotic systemic artery-to-pulmonary artery shunts who underwent transcatheter stenting at Rajaie Cardiovascular Medical and Research Center between 2011 and 2019. Result: Pulmonary blood flow increased in all the patients, and there were no significant complications or periprocedural deaths. Blalock–Taussig shunt stenting was successful in the early and late stages in all the patients with recorded data. Hematocrit was decreased by a median of 4 units (2–8 units). During the follow-up, 12 patients were rehospitalized, 4 underwent new procedures, and 4 died. Conclusion: Stenting an obstructed or stenotic systemic artery-to-pulmonary artery shunt is a safe and promising alternative to redo surgery with higher success rates and fewer complications compared with balloon angioplasty.
